“Analysis indicates that human error is identified as a causal factor in 80-90 percent of mishaps, and is present but not causal in another 50-60 percent of all mishaps, and is therefore the single greatest mishap hazard. Yet, simply writing off mishaps to ‘operator error,’ (or dismissing high accident potential conditions as something we just have to live with), is a simplistic, if not naïve, approach to mishap causation and hazard identification.
